REQUEST FOR DELETION OF DEDUCTION is a form needed by teachers to delete any unwanted double deductions and already paid deduction in the pay slip. In most cases the unwanted deductions were insurance policies and GSIS deduction. In this form, the teacher needs to fill up personal information regarding his/her Division and station codes. The teacher must affix his/her signature, further the name of the deduction in the pay slip and deduction code are also needed, in case of insurance policy- policy number is required with a stationary stamp signed by the manager. All you need to do is to submit the request form in your respective Regional Payroll Services Unit (RPSU) for proper action. It only need one month for your unwanted deduction to be deleted in your pay slip.
